this commit revamps the internal operations of the sharedfp components.
Specifically, it is focused around removing the second file_open
operation for shared file pointers. This makes the code more efficient.
Because of that, there is no necessity anymore for the sharedfp_lazy_open
mca parameter.

As we changed the ABI (forcing a major release), we can limit
the size of the predefined communicators by moving the collective
structure outside the communicator. This might have a minimal,
but unnoticeable, impact on performance. This approach has been
discussed during the January 2017 devel meeting.
