Deprecate the current OMPI-specific MPI_Info key definitions for
MPI_Comm_spawn and replace them with their PMIx equivalents. Issue a
deprecation/conversion warning as this is done. Also issue deprecation
warnings for options such as "ompi_non_mpi" that are no longer used.
Handle both cases where the user might pass either the PMIx attribute
name itself (e.g., "PMIX_MAPBY") or the string value of the attribute
(e.g., PMIX_MAPBY, which translates to "pmix.mapby"). This can only be
done for PMIx v4 and above, so protect that code.
Silence a couple of Coverity warnings and add a test along the way.

Do some code cleanup in the connect/accept code. Ensure that the OMPI
layer has access to the PMIx identifier for the process. Add macros for
converting PMIx names to/from strings. Cleanup a few of the simple test
programs. Add a little more info to a btl/tcp error message.

Will be replaced by PRRTE. Ensure that OMPI and OPAL layers build
without reference to ORTE. Setup opal/pmix framework to be static.

Update PMIx to latest master to get supporting updates. For
connect/accept (part of comm_spawn as well), lookup locality for all
participating procs on the node and compute the relative locality so it
can be used for MPI operations.

The Open MPI code base assumed that asprintf always behaved like
the FreeBSD variant, where ptr is set to NULL on error. However,
the C standard (and Linux) only guarantee that the return code will
be -1 on error and leave ptr undefined. Rather than fix all the
usage in the code, we use opal_asprintf() wrapper instead, which
guarantees the BSD-like behavior of ptr always being set to NULL.
In addition to being correct, this will fix many, many warnings
in the Open MPI code base.

The `ompi_comm_set` function never sets `NULL` to its first argument
`ncomm`. So `NULL` check is unnecessary in its callers. Furthermore,
`NULL` check may obscure a real return code when an error occurs
if the variable is initialized to a `NULL` value.
Also, `NULL` check is added in the `ompi_comm_set` function to
avoid segmentation fault in an out-of-memory condition.

The ompi tree should be runtime independent, but over time a few
ORTE depedent definitions and functions have escaped into the ompi
tree. I'm working on my own runtime so I've used this as an opportunity
to get rid of ORTE dependencies in the ompi/ tree. I still need to go
back and change orte to conform to the new world and these changes are
untested, but I can now compile (but not link) without orte so I'm
commiting this changeset.

As we changed the ABI (forcing a major release), we can limit
the size of the predefined communicators by moving the collective
structure outside the communicator. This might have a minimal,
but unnoticeable, impact on performance. This approach has been
discussed during the January 2017 devel meeting.

Internal, temporary libraries have not been renamed to limit the
scope of the patch to only what is needed.

This commit simplifies the communicator context ID generation by
removing the blocking code. The high level calls: ompi_comm_nextcid
and ompi_comm_activate remain but now call the non-blocking variants
and wait on the resulting request. This was done to remove the
parallel paths for context ID generation in preperation for further
improvements of the CID generation code.

This commit changes the way ompi_proc_t's are retained/released by
ompi_group_t's. Before this change ompi_proc_t's were retained once
for the group and then once for each retain of a group. This method
adds unnecessary overhead (need to traverse the group list each time
the group is retained) and causes problems when using an async
add_procs.

This commit removes the use of ompi_group_peer_lookup in the
ompi_dpm_mark_dyncomm function. The function now uses
ompi_group_get_proc_name which does not allocate an ompi_proc_t if one
does not already exist.
