will became the only pointer_array implementation (the orte_pointer_array
will be removed).

fact a free_list_item so instead of having a struct, use typedef
to make them equivalent. Modify the parallel debuggers support
in order to allow them access to the internal types even when
we have an optimized build.

opal_list_t, ompi_free_list_t so every time there is a modification
in one of these files (such as changing the way we allocate the
elements in the free list) the debugger interface have to
reflect these changes.
