(sometimes after the merge with the ORTE branch), the opal_pointer_array
will became the only pointer_array implementation (the orte_pointer_array

A new global structure (ompi_request_functions) holding all the pointers to the wait/test functions have been added. ompi_request_wait* and ompi_request_test* have been #defined to be replaced by ompi_request_functions.req_wait. The default implementations of the wait/test functions names have been changed from ompi_request_% to ompi_request_default_%. Those functions are static initializer of the ompi_request_functions structure.
To modify the defaults, a components 1) copy the ompi_request_functions structure (the type ompi_request_fns_t can be used to declare a suitable variable), 2) change some of the functions according to its needs. This is best done at MPI_init time when there is no threads. Should this component be unloaded it have to restore the defaults. The ompi_request_default_* functions should never be called directly anywhere in the code. If a component needs to access the previously defined implementation of wait, it should call its local copy of the function. Component implementors should keep in mind that another component might have already changed the defaults and needs to be called.

This commit essentially caches the invoking comm/win/file on the
ompi_request_t. This, paired with the req_type field, allows us to
retrieve the invoking MPI object and invoke the proper errhandler.
The patch is missing most updates for the MPI-2 one-sided stuff (i.e.,
the patch mainly fixes comms and files); I didn't really understand
that code and didn't want to hazard trying to figure it out when Brian
can probably do it much more quickly.
So #250 will still stay open, pending MPI-2 one-sided updates for this
stuff.

to let the PML (or io, more generally the low level request manager)
to have it's own release function (what was before the req_fini). This
function will only be called from the low level while the req_free will
be called from the upper level (MPI layer) in order to mark the request
as not used by the user anymore.
From the request point of view the requests will be marked as inactive
everytime we read their status (true for persistent as well). As
MPI_REQUEST_NULL is already marked as inactive, the test and wait functions
are simpler. The drawback is that now we have to change in the
ompi_request_{test|wait} the req_status of the request once we get it's
status.

have them as <project>

Any non-blocking P2P function should set the request to something else
then MPI_REQUEST_NULL, when passed a source/dest of MPI_PROC_NULL.
Introduce a new ompi_request_empty, which returns the
MPI_SOURCE=MPI_PROC_NULL, MPI_TAG=MPI_ANY_TAG, count=0 as specified by
the MPI-standard.
