If user sets HCOLL_EXTERNAL_UCM_EVENTS=1 then we try init opal
memory framework and register a mem release cb. Otherwise, rely on ucx.

Per MPI-3.1:8.7.1 p361:11-13, it's valid for MPI_FINALIZED to be
invoked during an attribute destruction callback (e.g., during the
destruction of keyvals on MPI_COMM_SELF during the very beginning of
MPI_FINALIZE). In such cases, MPI_FINALIZED must return "false".
Prior to this commit, we hung in FINALIZED if it were invoked during
a COMM_SELF attribute destruction callback in FINALIZE. See
https://github.com/open-mpi/ompi/issues/5084.
This commit converts the MPI_INITIALIZED / MPI_FINALIZED
infrastructure to use a single enum (ompi_mpi_state, set atomically)
to represent the state of MPI:
- not initialized
- init started
- init completed
- finalize started
- finalize past COMM_SELF destruction
- finalize completed
The "finalize past COMM_SELF destruction" state is what allows us to
return "false" from MPI_FINALIZED before COMM_SELF has been fully
destroyed / all attribute callbacks have been invoked.
Since this state is checked at nearly every MPI API call (to see if
we're outside of the INIT/FINALIZE epoch), care was taken to use
atomics to *set* the ompi_mpi_state value in ompi_mpi_init() and
ompi_mpi_finalize(), but performance-critical code paths can simply
read the variable without needing to use a slow call to an
opal_atomic_*() function.

If hcoll fails to create mpi derived type let's set zero_dte on this dtype.
This will save cycles on subsequent collective calls with the same derived
type since we will not try to create hcoll type again.

As we changed the ABI (forcing a major release), we can limit
the size of the predefined communicators by moving the collective
structure outside the communicator. This might have a minimal,
but unnoticeable, impact on performance. This approach has been
discussed during the January 2017 devel meeting.

* If hcoll is given a negative priority, but not enabled=0 then
the module is constructed, but then destructed before calling
it's query(). So the previous pointers are not initialized.
If we try to OBJ_RELEASE them in a debug build an assert will fire.
This commit adds some protection against that and initializes
the _module pointers to NULL.
- -check-shmem-params is OFF by default. It checks OSHMEM API params and will abort on bad input
- hcoll do not save fallback coll pointers for unsupported collectives.

Adds coll_hcoll_np mca parameter similar to that of fca component (defaults to 32). Those who use hcoll be aware that from now on the communicators less than 32 procs will run w/o hcoll by default. - Resolves fallback issue in case libhcoll runs out of allowed contexts. The solution is moving hcoll_context_create from comm_enable to comm_query. Shortly, comm_enable should never return OMPI_ERROR in the coll component with highest priority (hcoll). Otherwise the ompi coll_base_select will unselect the coll funtion pointers and module references leaving the communicator w/o coll pointer. This will cause the fail. Same behavior can be reproduced even with tuned if one would hardcore some "return OMPI_ERROR" into it's module_enable funtion. - Additionally, removed all the dead code under #if 0; removed unused variables (path for library, active_modules list) and classes (module list wrapper)

- Modifications to coll/hcoll component related to the changes in the libhcoll API.
Now, hcoll_destroy_context accepts one more parameter that indicates if the context was
really destroyed as a result of the call.
This new "non-blocking" context destruction fixes hang discovered in IMB with mcast enabled.
- Clean up all the left contexts (if any) on the comm_world destruction.

1. Change in rte api implementation: now comm_world used to do p2p.
This allows to not worry about other comms being destroyed.
2. added a notification mechanism with a help of which runtime can say libhcoll that RTE api can not be used any longer.
pass a pointer to a flag, and its size to libhcoll.
The flag changes when the RTE is no longer available.
Currently this flag is just ompi_mpi_finalized global bool value.
