The Open MPI code base assumed that asprintf always behaved like
the FreeBSD variant, where ptr is set to NULL on error. However,
the C standard (and Linux) only guarantee that the return code will
be -1 on error and leave ptr undefined. Rather than fix all the
usage in the code, we use opal_asprintf() wrapper instead, which
guarantees the BSD-like behavior of ptr always being set to NULL.
In addition to being correct, this will fix many, many warnings
in the Open MPI code base.

In many cases, this was a simple string replace. In a few places, it
entailed:
1. Updating some comments and removing now-redundant foo[size-1]='\0'
statements.
2. Updating passing (size-1) to (size) (because opal_string_copy()
wants the entire destination buffer length).
This commit actually fixes a bunch of potential (yet quite unlikely)
bugs where we could have ended up with non-null-terminated strings.

This commit expands the effect of the MCA parameter `opal_abort_delay`
to the OPAL signal handler. This allows attaching of a debugger on
segmentation fault etc. before quitting the job.
The sleep code is moved to the `opal_delay_abort` function from the
`ompi_mpi_abort` and `oshmem_shmem_abort` functions for code cleanup.

This commit does two things. It removes checks for C99 required
headers (stdlib.h, string.h, signal.h, etc). Additionally it removes
definitions for required C99 types (intptr_t, int64_t, int32_t, etc).

This commit fixes a bug when opal_error_init is called with the same
values multiple times. If opal_error_init is called too many times it
will start failing with OPAL_ERR_OUT_OF_RESOURCE. To fix the problem
check if an existing convertor matching the requested one and return
that one instead.

Roll in the ORTE state machine. Remove last traces of opal_sos. Remove UTK epoch code.
Please see the various emails about the state machine change for details. I'll send something out later with more info on the new arch.

Add some new proc/job states
Rename a constant to reflect coming change - remove the arbitrary difference between restarting a proc locally and relocating it to another node in terms of the number of restarts allowed.
Add pretty-print of signals for "proc aborted due to signal" reports.

This means that the converters (opal_err2str, orte_err2str) can now
return NULL as a "silent error". The return value of opal_err2str_fn_t
is the status of the operation (OPAL_SUCCESS or OPAL_ERROR).
This fixes the "Unknown error" message issues on the trunk.

(OMPI_ERR_* = OPAL_SOS_GET_ERR_CODE(ret)), since the return value could be a
SOS-encoded error. The OPAL_SOS_GET_ERR_CODE() takes in a SOS error and returns
back the native error code.
* Since OPAL_SUCCESS is preserved by SOS, also change all calls of the form
(OPAL_ERROR == ret) to (OPAL_SUCCESS != ret). We thus avoid having to
decode 'ret' to get the native error code.
