Make openib btl a bit more resillient in the face of driver errors --

return OMPI_ERR_UNREACH if the port returns an invalid speed or
width.  OMPI_ERR_VALUE_OUT_OF_BOUNDS is reserved for when we exceed
the number of allowable BTLs.
